LIABILITY WAIVER AGREEMENT

Assumption of Risk

I acknowledge that my child’s participation in Overtime Basketball Academy is voluntary and carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. I assume these risks and agree to be held fully accountable for any injuries that might occur in my child’s involvement with Overtime Basketball.

 

Release of Liability/Indemnification

I therefore hereby release Overtime Basketball Academy,  its coaches, leaders, employees, volunteers, and agents from any claim that my child may have or that I may have against them from any and all liability from personal injuries, illnesses or other claims arising from my child’s participation in Overtime Basketball’s programs and classes. I further release any gym or other facility used or occupied by Overtime Basketball Academy for any of the claims stated above. 

This release of liability shall include (without limitation) any claims of negligence or breach of warranty. 

 

 Visual records

On occasion, this company takes photographs, makes audios or videotape recordings,  of children and/or adults involved in activities. I consent to the distribution or display of audio or visual record of my child, for the purpose of any use deemed as appropriate by Overtime Basketball’s agents.

The Overtime Basketball participants or their legal representative must agree to this waiver and thus understand that it shall be binding upon them, their family, legal representatives, successors and assigns.
